Saskatoon CPAP can arrange a Home Sleep Test through our office. If you have a family physician, complete the request form and we will request a referral for testing on your behalf. If you do not have a family physician, take a copy of the Printable PDF Referral Form to a walk in doctor to be completed. Saskatoon CPAP requires a valid referral to arrange the Home Sleep Test.
